How the rebate works.

You are able to combine your Altosid^® and Zenivex^® product purchases to meet rebate levels. You have until January 31, 2013 to submit your rebate form for purchases dated January 1, 2012 to December 31, 2012. Submit your form at any time, and remember that the more you purchase, the greater the rebate!

How the payback works.

Rebate levels have been expanded and are now based on total 2012 purchases (excluding taxes) of all Altosid^® and Zenivex^® products.

Your Cumulative Purchase Rebate % $10,000 to $24,999 1.0% $25,000 to $74,999 1.5% $75,000 to $149,999 2.0% $150,000 to $299,999 3.0% $300,000 to $499,999 4.0% $500,000 + 5.0%

How to get your rebate.

1. Print and complete the form located in the rebate program brochure PDF below.
   
2. Attach all relevant invoice copies (important)!
   
3. Place in envelope with required postage and send to:

Rebate program is available only in the U.S. for municipal, county and state agencies and U.S. commercial organizations that purchase Altosid® and Zenivex® products from an approved U.S. agent. Purchases must be made between January 1, 2012 and December 31, 2012. Rebate form must be postmarked by January 31, 2013 accompanied with appropriate invoice(s) to be eligible.
